I've used Dan's Chimney service for years.  He arrives promptly, does a nice job and cleans up afterward.

I have also used Dan.  He is expensive, but gets the job done.  He cleaned, repointed, and capped both my chimneys for about $2,000.  I have also used Chimney Joe Dvorak.  They reflashed both chimneys to stop leaks, cleaned and repaired the gutters, and did a little work on my eaves for about $700.  So far, so good.
